- Qualifying products must be purchased
- To receive your rebate refund, you must: (1) Complete & sign above form (Request form may not be altered). (2) Include proof of qualifying purchase by including a legible, complete photocopy of your entire sales receipt showing your name & address and all product/order info. Please understand, order acknowledgments, packing slips or purchase order copies do not qualif.y (3) Send form and photocopy to address on rebate form postmarked no later than March 1, 2011.
- A separate form and separate copies must be completed for each separate rebate.
- Offer is not valid with the purchase of used, refurbished, demonstration or clearance equipment.
- Resellers, distributors, dealers, brokers are not eligible.
- Rebate check will be mailed via US Mail to the address provided on this form (checks must be sent to addresses within 50 United States).
- Please allow six to eight weeks from the date your claim is processed to receive your check.
- Offer good for sales in USA only.
- Rebate and invoice/receipt name and address must be the same.
- Machine must be paid in full before February 28, 2011.
- Lost, late, or misdirected mail is not the responsibility of HIX Corporation or its agents.
- This offer is void where prohibited or restricted by law.
- HIX Corporation is not responsible for printing errors.
- Invalid submissions will not be returned and become the property of HIX Corporation.
- HIX Corporation reserves the right to change the Terms and Conditions or end the offer at any time without notice.
- Keep copies of these Terms and Conditions, your completed form, proof(s) of purchase, and serial numbers of the machine(s) for your records.
